In conjunction with providing professional care management services to families, CARE has developed a new website to help families determine to what extent and what type of care an elder may need. In the informative “Frequently Asked Questions” section, families will be able determine if they may need the assistance of a professional care manager. A complete description of the extensive services that CARE provides is also helpful for site visitors who are trying to establish whether outside care and resource assistance would be highly beneficial to an elder and their family.According to Kate Granigan, MSW, LICSW, CARE’s founder, Executive Director and Managing Partner, “Enabling our geriatric clients to attain their maximum functional potential is our primary objective. Our perspective is that of a social worker, where we help our clients maneuver through systems and obtain the best possible overall care for optimal quality of life. It is different from the nursing perspective which deals less with systems and more with ‘hands-on’ care.”
CARE strives to deliver care coordination and support services that will not only meet an elder’s needs, but do so in such a way that preserves dignity. Among the many ways CARE assists elders is by finding and managing home based services such as in-home caregivers or nurses; by acting as an advocate so elders receive the benefits they deserve; and by serving as an elder’s “eyes and ears” at medical appointments.
The agency’s new website will better acquaint elders and their families with the type of advocacy and services CARE provides as licensed, professional care managers.
Dedicated to helping elders living in their own homes or in assisted living facilities, CARE’s managing partners joined together in 2004 to fulfill a community need for geriatric care managers who are licensed professional human service providers. All of CARE’s staff are master’s degree level Board Certified Clinical Social Workers and Nurses and are experienced specialists in providing care to elders. The agency establishes a new professional level of services designed to screen, arrange and monitor in-home help or other services for elders. CARE can also provide crisis intervention, and can act as a liaison for families who live far away.
